The problem
A doctor's patient intake and follow-up were entirely manual — long paper forms, scattered notes, follow-ups tracked from memory. When a patient returned, finding their previous history meant digging through stacks of paper, wasting time and risking continuity of care.
What I built
A clinic system where any physician can register from anywhere, create a profile and start working with no training curve. Digital intake forms replace paper, patient records carry full history, and follow-ups are scheduled by the system rather than remembered.
What it does
- Self-service physician registration and profiles
- Digital patient intake forms
- Full patient history, instantly retrievable
- Structured follow-up scheduling
- Responsive interface for consulting-room use
The outcome
Doctors spend less time on paperwork and more on patients. Follow-ups became reliable rather than memory-based, patient histories are instantly available, and onboarding a new doctor takes no training.
